When completing the intake form, be sure to provide all requested information. We will not respond to incomplete surveys. If you have any questions, note them in the survey.
We will follow up with you to sign an agreement and schedule your initial sessions.
We will follow-up to schedule your free initial consultation, during which we will co-develop the scope of work and budget.
ALL PARTIES must complete their own intake forms.
